    Protocol for hydroxychloroquine screening optometry

"Look at everything—every time" is a mantra many of us have humming in the background as we go through our clinical day. Accordingly, providing screening examinations for high-risk medications, such as hydroxychloroquine, is a natural service for our profession.

Abstract. Background The American Academy of Ophthalmology recommendations on screening for chloroquine CQ and hydroxychloroquine HCQ retinopathy are revised in light of new information about the prevalence of toxicity, risk factors, fundus distribution, and effectiveness of screening tools. Humphrey visual field testing of the central 10-2 revealed incomplete paracentral annular scotoma. Subsequently, hydroxychloroquine was switched to sulfasalazine. CONCLUSIONS The 2016 American Academy of Ophthalmology guidelines for hydroxychloroquine retinopathy were revised to reflect new dosing and care guidelines for early detection of retinal toxicity and to minimize the extent of irreversible vision loss. In 2002, the American Academy of Ophthalmology AAO established ocular examination guidelines for screening patients on Plaquenil therapy. Testing included a comprehensive eye exam that consisted of an assessment of the posterior segment with careful evaluation of associated macular changes or signs of retinal disease. 4 Baseline fundus.

The retinopathy is hallmarked by parafoveal changes within the retina. Hydroxychloroquine is an analogue to the much older drug chloroquine. Hydroxychloroquine is most frequently used for the management of lupus and rheumatoid arthritis; however, it is now being applied to treatments in adjunctive cancer therapy, diabetes, and heart disease.

The AAO published dosing and screening recommendations for chloroquine and hydroxychloroquine in 2016, revising previous recommendations published in 2011. The current maximum daily HCQ dose recommended by the AAO is 5 mg/kg of real weight. The 2011 guidelines suggested a maximum dose of 6.5 mg/kg of ideal weight. Generally, the plaquenil eye exam is something everyone here has done. These are the patients where you need to knock out that extra 10-2 in your exam.except that may not be true anymore. What is plaquenil used for? HCQ is used for RA, Lupus and for SS among other conditions. RA is the most common of these at ~1% of the population. My Take on New Ocular Screening Guidelines for Plaquenil By Sergio Schwartzman, MD 12 October 2016 A recent article published by the American Academy of Ophthalmology AAO presented new recommendations for screening of patients being managed with hydroxychloroquine HCQ.
