Most importantly, you help support continued development of this site. The majority of cases of retinotoxicity have occurred in patients that have had a cumulative dose exceeding 1000g of hydroxychloriquine (Plaquenil). This level is reached in about 7 years with the most common daily dose of Plaquenil, 400 mg/day (200 bid).

Plaquenil hydroxychloroquine is in a class of drugs called disease-modifying anti-rheumatic drugs, which are used to decrease inflammation, pain and joint damage. The vast majority of patients on Plaquenil get absolutely no eye problems and you should not be unduly worried or concerned. If you have a good response in your arthritis to the Plaquenil then the benefits of a Plaquenil almost certainly outweigh the small risks to your eyes.

  7. Dr. Marmor, who chaired the Academy’s screening guidelines committee, said the goal in Plaquenil screening is to catch changes at a very early stage when there’s just a minimal amount of loss of vision. “Then, when you stop the drug, the damage won’t progress enough to cross the fovea and affect reading vision.”

Plaquenil, What it is and how it Affects Your Eyes. Often times when a patient is diagnosed with systemic conditions such as Rheumatoid Arthritis, Sarcoidosis, Lupus, and other autoimmune disorders, a physician will prescribe Hydroxychloroquine, commonly known as Plaquenil. What does Plaquenil/hydroxychloroquine do to your eyes? The retina is the film in the back of your eyes. CQ and HCQ can both cause maculopathy or retinopathy. Prior to modern diagnostic techniques, your eye doctor was looking for bull’s eye maculopathy with a dilated exam. He has Sjogren's Syndrome. He became very ill, and was diagnosed w/ viral meningial encephalitis. We were much later told that the Plaquenil had "lowered his tolerance to infection, as it is also an immuno-suppressant." In essence, the medicine allowed the viral infection to cross the barrier between his brain and blood/lungs.

My Take on New Ocular Screening Guidelines for Plaquenil. The study findings revealed that when hydroxychloroquine was used at doses 5mg /kg, the risk of retinopathy was 1% in the first 5 yrs. 2% in 10 yrs. 20 % after 20 yrs. There was a dose response, such that those receiving doses greater that 5 mg / kg had the greatest risk of retinal toxicity.
